On the day of your appointment, we ask that you bring two things:
1.A valid form of ID, such as your driver's license or passport, and
2.If insured, your health insurance card.
Minors (individuals under the age of 18) are also required to bring their parent or guardian. If you are coming in to be fit for orthotics, we request that you bring the shoes that you plan to wear the orthotics in. Depending on the device prescribed by your doctor, our representative or brace fitter may give you additional instructions when you make your appointment.
