On the day of your appointment, we ask that you bring two things:

1. A valid form of photo ID, such as your driver's license or passport, and
2. If insured, your health insurance card.

Minors (individuals under the age of 18) are also required to bring their parent or legal guardian.

If you are coming in to be fot for orthotics, we request that you bring the shoes you plan to wear your orthotics in.

Depending on the device prescribed by your doctor, our representative may give you additional instructions when you make your appointment.
